DePaul's Woodcrest Commons Senior Living Community is looking for a Registered Nurse to join our team! The Director of Clinical Services is responsible for providing coordination and oversight for the LHCSA/ALP program in conjunction with administrative staff and other service agencies. The Director of Clinical Services also participates in an interdisciplinary team to support the goal of providing excellent quality care to all residents.

NYS Registered Nurse (RN) with a minimum of 3 years of nursing experience with one year of experience in LHCSA, CHHA, or LTHHCP are encouraged to apply!

Job Duties:

Coordinates the overall clinical program of the LHCSA / ALP in conjunction with the Administrator, Resident Care Director (RCD) Memory Care Director (MCD) Residents' Physicians and other service agencies.
Completes nursing assessments of residents referred for services, identify residents' needs, consult with residents' primary physicians and initiates care plans.
Facilitates on-going re-assessments of residents' needs as determined by changes in resident's status or as required by New York State Health Department regulations and agency policy.
Assists RCD with service coordination.
Orients and supervises home care staff in conjunction with the RCD.
Establishes annual in-service training calendar, trains and monitors staff attendance.
Completes competency evaluations and annual performance evaluations for all home care staff.
Evaluates and follows-up with all resident or employee grievances.
Evaluates and follows-up with all resident or employee accidents or incidents.
Counsels and disciplines all home care staff as required.
Coordinates the Quality Improvement Committee as required by regulation.
Acts as the on-call nurse, as required, to respond to clinical problems.
Assists Administrator / Case manager with marketing activities, as requested.
Other duties as directed by the Administrator.


Is responsible for assuring that resident care practices / policies are followed.
Assures that Resident Care Staff abides by company policies and procedures.
Provides feedback through appropriate chain of command recommending any needed change in policy.

Planning and Organizing

In collaboration with the Administrator and the Resident Care Director decides the staff mix (PCAs, HHAs).
Provides sufficient staff to assure uninterrupted service to residents at all times.
Implements and maintains a system for assuring all appropriate staff participate in required in-servicing.
Implements and keeps current a system that identifies due dates for resident's medical evaluations & UAS updates and Care Plans.


Obtain by recruiting, interviewing, verifying references, obtaining criminal checks and employing the number and categories of Resident Care personnel needed.
Explain personnel policies and individual duties to each new employee and assist with orientation to the facility and his/her job.
Completes and maintains thorough supervision documentation for each HHA and PCA.

4. Directing

Monitor, by daily rounds, the work of all ALP staff.
Establish and maintain individual personnel records on each HHA and PCA in conjunction with RCD.
Evaluate the work of each ALP staff person and periodically discuss the evaluation with the employee.
Provide opportunities for regular staff meetings.
Counsel and develop employees as the need is apparent; dismiss from employment as policy and need indicate (with the approval of the Administrator and HR department).

Resident Care

Ascertain that each resident has a care plan and that the ALP staff is aware of and following it.
Insure that Staff are providing medications, diets, activities of daily living and safety measures for ALP Residents.
Schedule conferences with resident and family, in conjunction with the RCD, when necessary to discuss policies and concerns relating to ALP issues.
Refer residents and families to appropriate community agencies as necessary.
Make personal rounds of entire ALP census on a daily basis to be informed of and involved with the resident's problems and to make sure staff is rendering proper and adequate care.
Schedule and conduct regular conferences with the staff to explore and explain difficult resident behavior and resolve problems of resident care.
Completes and maintains thorough documentation for each ALP Resident on all aspects of care including the care that is completed by outside providers.

Resident Records

Review controlled substances weekly to ascertain that they are properly counted and documented from shift to shift in conjunction with the RCD.
In conjunction with the RCD, perform record audits of a sample number of Resident charts monthly to insure proper charting and documentation of care; confer with SIC's not properly documenting.
Respond to pharmacy reports, in conjunction with the RCD.
In conjunction with the RCD, respond to medical providers' recommendations and any other resident care issues.
Respond to Corporate Quality Assurance reports in conjunction with the RCD as required.

Other Duties

Assist in maintaining quality assurance data and reports with the Corporate Quality Assurance Consultant.
On-Call Duty.
Other duties / responsibilities as assigned by the Administrator.

Ability to communicate with residents and others in the English language.
Ability to read and write English.
Ability to lift up to 30 lbs. without mechanical assistance.
Ability to visually observe resident.
Ability to audibly hear call bells, emergency systems, verbal needs expressed by resident, and sounds related to physical assessment (B/P, apical heart rate).
Ability to turn, stoop, bend, and stretch in order to assist residents.
Ability to stand and walk for prolonged periods.
Ability to recognize and respond to emergencies, e.g. evacuation of residents and visitors in the event of fire.
Ability to push the medication cart down the hallway without assistance.


Graduate of an accredited School of Nursing.
Valid and Current New York State Registered Nurse License and Registration.
Minimum of three years nursing experience with one (1) year of home care experience in a LHCSA, CHHA, or LTHHCP preferred.
Must possess a valid New York State driver's license that meets DePaul's clean driving record policy.

Work Environment

Well-lighted, air-conditioned / heated environment; may be exposed to residents with communicable diseases or confused residents who become combative. May be required to work outside on occasion when required by resident care needs (i.e., resident walks or other outdoor resident activities).

Protective Equipment Used/Equipment Needed

Latex gloves, goggles for standard precautions. Office equipment (copier, computer, fax machine, telephone), fire extinguisher, wheelchairs.
